Comparative analysis of 2- and 3-dimensional laparoscopic donor nephrectomy

Background. This is the first report on 3D-laparoscopic donor nephrectomy performed in the Kazakhstan and CIS countries. The report aim was to present the results of the first experience of hand-assisted 3D-laparoscopic donor nephrectomy (3D-HLDN) and compare them with the hand-assisted 2D-laparoscopic donor nephrectomy (2D-HLDN) outcomes within a single center.
Materials and methods. Twenty 3D-HLDN and twenty 2D-HLDN performed were analyzed. The technique of all 40 HLDN was identical. Statistical differences between the two groups were considered significant at p<0.05.
Results. The baseline characteristics in both groups were statistically comparable (P > 0.05). All 40 donors underwent left-sided nephrectomy. Donors with 3D-HLDN had better data compared to 2D-HLDN: shorter warm ischemic time (P < 0.05), shorter operative time (P < 0.05), lower blood loss (P < 0.05). There were no conversions and no major complications by Clavien-Dindo in both groups. The average drainage duration and postoperative hospitalization were significantly shorter in the 3D-HLDN (P < 0.05). The differences of the mean postoperative creatinine and GFR in both groups of donors statistically were not significant.
Conclusion. 3D-HLDN beneficial than 2D-HLDN in the form of short warm ischemic time, a low volume of blood loss, reduced drainage duration and postoperative hospitalization stay. Postoperative complications and the functional condition of the donor’s kidney did not depend on the type of performed LDN.
