Introduction: Orthotopic heart transplantation remains the gold standard treatment for patients with end-stage chronic heart failure. Annually at the Shumakov National
Medical Research Center of Transplantology and Artificial Organs
Made more than 200 heart transplants in adult patients with CHF, which in turn has led to an increase in the number of patients with a transplanted heart requiring outpatient monitoring at different periods after heart transplantation
Objectives:  evaluate personalized follow-up the long-term results after heart transplantation in 1436 patients in the consultative and diagnostic department
Methods: The study included 1436 outpatient aged 18 to 70 years, who underwent heart transplantation from January, 2008, to December, 2022. We assessed the incidence of long-term complications after heart transplantation, such as chronic allograft vasculopathy (CAV), renal insufficiency, post-transplant diabetes and malignancies.
Results:  during outpatient follow-up were: hypertension (48.65%), post-transplant diabetes mellitus (7.24%), nephropathy (35.97%), and malignant neoplasms (4.2%). Renal insufficiency renal failure was diagnosed in 17% of patients at year 1 and 5 of follow-up. Recipient survival, excluding in-hospital mortality, was 96.5%; and 88.0% at year 1 and 5 of follow-up, respectively
Conclusion: The personalized approach for outpatient after heart transplant will improve recipient survival in the long-term post-HT period.
