Introduction: Death with a functioning graft (DWF) has increased over the decade, due to older patient population with multiple comorbidies and use of multiple immunosuppressive treatment. The ratio of mortality due to infectious causes has also highly increased.
Method and Aims: Our objective was to identify the main causes of mortality of renal transplant patients in our hospital and described the frequency of death with functioning graft (DWF), as well as the main causes of mortality in our patients.
Study population -137 patients
Study period – 1 Jan 2023 to 31 Dec 2023
Study method – Retrospective observation study
Study place - Nizam Institute of Medical Sciences , Punjagutta , Hyderabad
Results:

During the follow-up period of 1 year, death with functioning graft (DWF) was the single most commonly observed cause of allograft loss, (N -11 grafts, 8.02% of all transplants).
The frequency of death before discharge was 1.5% (N=3)
The first cause of death was infections (N -9, 81.81%), followed by cardiovascular (N- 2, 18.19%). In that, major infectious foci was respiratory tract infection in around 77.7% cases.
Total graft loss in our study was 11.6 % (N -16)
The remaining grafts were lost due to a variety of causes, including rejection (N-2), reccurence (N-1), graft mucormycosis (N-1) and TMA (N-1).
Conclusions: In 1 year follow up, patient´s death with a functioning graft (DWF) due to infection was major cause of allograft loss. So, judicious use of induction and maintenance immunosuppression drugs, vaccination and antibiotics prophylaxis to prevent infection is paramount important.
